Patients who are waiting to see Dr. Jiang should complete New Patient Questionnaire to expedite your appointment.

Patients who saw Dr. Jiang in consultation and who have had or are undergoing investigations should inquire about future appointments or results by calling the office. See Contact information page. We have noted that a large proportion of patients do not return our call or voicemail for follow up. Hence, we ask that you contact our office instead to schedule an appointment at your convivence.

Please be kindly advised that all surgical procedures (hernias, gallbladder removal, etc.) and endoscopy (incl. colonoscopy and gastroscopy) are arranged by the Hospital Booking Office. This is a Hospital Policy. Hence, our office will not be able to your procedures. We will really appreciate your understanding on this.

If your symptoms have worsened, please notify our Office and/or your Family Physician for reassessment or re-prioritization of your treatment as soon as possible.

To facilitate the care of all patients and to reduce unnecessary wait time, if you have decided NOT to undergo a procedure including, colonoscopy, please contact Dr. Jiang's office or the Booking Office immediately to cancel it. We will appreciate it if you can contact us at least 7 days in advance. You must also inform your GP of your decision to cancel. Similarly, if you are sick, e.g. cold, COVID, etc., please kindly inform the Booking Office immediately to be rescheduled. It is not advisable to undergo surgery while you are suffering or recovering from a respiratory infection.

Access to Your Health Information

Please note that Dr. Jiang and his staff, locum or learners (if applicable) will need access to your health information stored in PharmaNet, PowerChart, CareConnect and any paper medical charts, for the purposes of providing treatment or care to you during your referral.

Referral means one physician (e.g. your GP, emergency room physician, another physician whom you visited, etc.) has asked Dr. Jiang to see you for a surgical problem, such as gallstones, hernias, breast cancer, etc. Throughout the duration of this Referral, you are considered Dr. Jiang's patient. You are no longer his patient once you are treated and 'discharged' from his practice. Hence, the duration of the Referral can be variable.

PharmaNet is a BC Provincial database that contains all your prescribed medications in BC. For more information, please visit Provincial website.

CareConnect is another BC Provincial secure, view-only Electronic Health Record. "It offers healthcare providers access to an integrated, provincial view of patient-centric information available 24/7 to support the delivery of patient care. CareConnect is widely used within six Health Authorities across the province, with over 66,000 healthcare professionals enrolled to support direct patient care." No provincial website available.

PowerChart (formerly known as Cerner) is an integrated Electronic Health Record specific to Vancouver Island Health Authority (VIHA). All your results (bloodwork, X-rays, CT scan, etc.) and visits records (vital signs, consultations, surgery, etc.) to any VIHA facility (entire Island Health from Victoria to Port Hardy) are store in PowerChart. This greatly facilitates your care by any healthcare provider on the Island. Note: LifeLabs results will not appear in PowerChart and will require additional access via CareConnect.

Paper medical charts include all your paper-based medical records stored at Campbell River Hospital, copies of your old records delivered to Dr Jiang's office by your GP or yourself.
